This affidavit/declaration shall remain in effect until revoked in writing by the undersigned and is delivered to the
Southwest Livestock Exchange, Uvalde, TX.
The undersigned hereby certifies, to the fullest extent of their knowledge and belief, as of the date of shipment or delivery, that none of the livestock shipped or delivered to Southwest Livestock Exchange will, on such date, be adulterated within the context of the Federal Food, Drug, and Cosmetic Act. Specifically, it is affirmed that none of the cattle or other ruminants will have been administered feed containing protein derived from mammalian tissues (for example, meat and bone meal) as defined under 21 CFR 589.2000, nor will any of the livestock possess illegal levels of drug residues. This certification shall persist in full legal effect until explicitly revoked in a written notice by the undersigned seller, with such notice of revocation to be duly delivered to Southwest Livestock Exchange.
Pursuant to the regulations of the United States Department of Agriculture (USDA), which recognize this affidavit as an official record of Country of Origin, the undersigned solemnly attests, based upon direct knowledge, customary business records, or corroborative producer affidavits, that all livestock mentioned herein or in any related communications pertaining to this transaction are of United States origin. In the event that the origin of the aforementioned livestock deviates from that stipulated herein, the undersigned commits to promptly notifying the purchaser or their designated agent of such change. This affidavit or declaration shall retain its validity and remain in force until such time as it is formally rescinded by a written revocation issued by the undersigned, which must then be duly submitted to Southwest Livestock Exchange (in the capacity of agent/buyer).
